At Urologic Physicians, PA, we are a team. The team working on your behalf includes:

Physicians
We believe in the value of a close patient-physician relationship. To provide continuity of care, in most cases you will see one specific physician for your care. Although one physician will supervise your care, you benefit from us being a group practice. Members of the group consult with each other to encourage consistent, advanced and conscientious care. Because our physicians sub specialize, you may be referred by your UPPA urologist to a partner whose subspecialty suits your needs. If your regular doctor is absent, another of our doctors is always available. In the event of an urgent situation, it may be necessary and advisable for you to see the most available doctor.

Nursing staff
Our nursing staff is skilled and experienced in the field of urology. They will guide you through the urologic procedures, provide teaching and information, and prepare you for further treatment or follow-up. Our nursing staff regularly attends continuing education offerings to further complement their urologic background and skills, they are excellent resources when you have questions.

Patient Coordinators
Our team of patient coordinators plays an important role in your care. They will greet and assist you when you come to the office for your appointment. They will schedule future appointments, laboratory, and radiology testing. In addition, a patient coordinator will assist you through the entire scheduling and preparation process should it be necessary for you to undergo surgery. This team is instrumental in helping you to resolve your concerns and questions and in referring you the appropriate resources in the clinic.

Medical Records Staff
Our medical records staff will assist you with your requests for medical records. They will also attend to your prescription refill requests.

Business Office
We regard assisting patients with financial matters related to their care as an extension of the medical service itself. Our business office staff is trained and prepared to assist you with the many questions that may arise regarding your insurance, account information, fees and other billing concerns. We recognize that questions regarding such matters are often of concern to our patients, and our business office staff stands ready to help you in this regard. If you have questions about your insurance coverage and our services they are always happy to help.

Administrator
The administrator is in daily contact with our physicians, clinic team supervisors, and team members. Clinic procedures are reviewed regularly, and top priority is given to patient interests, services, and concerns. Should a situation of extreme concern or sensitivity arise, the administrator is available as a resource to help patients address any such concerns. The primary goal of the clinic administrator is to coordinate the operations of the clinic, so that we offer optimal service to our patients.

Appointments
To reduce your waiting time and aid in office efficiency, our practice is organized on an appointment system. Please mak your appointments as far in advance as possible. We usually have our schedules set up at least six months out. We ask that you advise us regarding any delays or the need to cancel your appointment. If your telephone number has changed since your last visit please let us know in case we need to reach you for an appointment change. We will contact you as soon as possible if it is necessary for to reschedule your appointment due to urgent or emergency circumstances.

When you arrive for your appointment we will verify your demographic information and insurance. We will also request that you update your signatures on a yearly basis.

At times, blood tests and radiologic studies may be important tools to help monitor your urologic condition. Tests that are done at other facilities will take several days to process before we receive your report. We will contact you about test results that require immediate attention. The patient coordinators will schedule your tests so that the results will be available for review at your next appointment. If you have had testing ordered by another physician (your primary care MD or another specialist) please let us know. We will either ask you to call them and request copies or we will make a request on your behalf.

Prescription Refills
Refill requests by telephone will be fulfilled during office hours only. We require 48 hours for turnaround time in order for your request to be processed. The most efficient way to have your medication refilled is to call your pharmacy and request that they call us for your refill.

We DO NOT fill narcotic prescriptions during evening and weekend hours unless the physician on call is able to see you at a hospital emergency room.

We DO NOT refill ED medications during evening and weekend hours.

New Prescriptions
We are unable to keep up with ever changing formularies (approved medication lists) offered by the insurance plans. If your pharmacy calls to alert us that we have written a prescription that will result in a higher out of pocket expense for you we will substitute the formulary approved option if it is truly a substitute for the medication we have written. If you try the formulary medication and find that it does not work for you or results in side effects, we will then attempt to have a prior authorization approved.

Our Edina and Chicago Avenue offices are located next to parking garages. We do not validate parking in these locations.

Medical Records Requests
We are happy to fulfill your medical records requests. We are limited by the HIPAA laws as to where and to whom we can forward your medical record. We may send your records to the address you have given us (your home address of record). We cannot fax your records to a number that you have given us over the phone. You will have to make a request to fax in writing. We are allowed to share your medical record with your referring physician (usually your primary care physician). If you would like us to forward your medical record to a physician other than the referring physician, your request must be in writing. Billing
To maintain our ability to run our business, we depend upon your prompt payment. We invite you to discuss charges and fees prior to being cared for. The fees for new patient visits will be slightly higher than the fees for follow up visits. This is to cover the additional time needed to complete a thorough and current history, set up your file, and communicate with your referring physicians. Copays and Out of Pocket Expenses
If your insurance coverage requires you to pay a copay, we ask that you be prepared to pay it at the time of service. We accept cash, check, Visa and Master Card. If you are unwilling or unable to make your copay at the time of your appointment we will ask that you reschedule. If you have a percentage out of pocket expense which is determined by the insurance company, we ask that you pay upon receiving your first statement. If you have any questions about your statement please do not hesitate to call our business office at 952.920.2046. If you have a percentage out of pocket expense, and have not paid in a timely manner in the past, we will estimate your out of pocket expense at the time of service and will request that you pay at the time of your appointment.

Insurance
You may have health insurance to help you meet the cost of medical service. However, the basic responsibility for payment is yours. Your insurance contract defines the extent to which that company will reimburse you. We do not know the requirements of individual plans, and usually cannot tell you whether certain services are covered under your insurance plan.

We ask for your cooperation in keeping us informed regarding your insurance information, so that we may help you to carry out the many requirements that are part of these insurance plans. These may include authorization for admission to the hospital and certification for certain procedures. You must bring with you or arrange to have a referral form sent to our office for every visit, if required by your health plan. If you are unsure whether this is a requirement of your plan, please call them to find out. Make sure to bring your insurance health card with you each time you are seen in the office. We reserve the right to reschedule your appointment if we are unable to verify your insurance at the time of your visit.

We do not participate in all insurance plans. We also do not usually know whether our ancillary providers (radiology facilities, laboratory facilities) participate with your plan. If you are unsure whether a facility at which we have scheduled testing is a participating provider with your plan, please call you insurer. This could save many dollars in out of pocket cost. We ask that you make inquiries into the rules of your plan and participating providers through the number on the back of your insurance card.

Hospital Affiliations
We provide services at the following hospitals:

We will work with you and your primary care physician in determining which hospital is most appropriate for you if hospitalization is necessary. This will be determined in cooperation with your physician, by the specific medical care needed, and may be affected by the contractual agreement of your health plan.

New Patients
Thank you for choosing Urologic Physicians. After making your first appointment, our new patients usually receive a packet of information in the mail which includes a letter that confirms the date and time of your appointment, a map to the office, a demographic questionnaire and a health history form. Please have your forms completed before you arrive at the office. If you do not receive the forms, please report to the office 15-20 minutes prior to your appointment time. We also request that you bring your insurance card or cards. If we cannot verify your insurance, we may ask you to reschedule. We hope that your experience at our clinic is a good one.
